New Patient Registration Forms

We attempt to see you at your appointed time as much as possible since we believe your time is as valuable as ours. If you would like to save some time, download our New Patient Registration form and our Treatment Goals for Dental Rehabilitation form if needed. Please fill them out and fax or mail it to us after you complete the form(s). You can email it if you are able to scan the form into your computer. By completing these in advance, this will allow your chart to be prepared ahead of your first appointment and insurance benefits determined. If not this may delay completing your initial evaluation.
